POSITION SUMMARY:

Granite United Way, in collaboration with the NH Care Connections Network, the NH Department of Health and Human Services, and Unite Us, is hiring two Regional Engagement Coordinators to join our team. Successful candidates will be assigned to a location based on availability and preference.

A Regional Engagement Coordinator is passionate about public health and care coordination. Unite Us supports a Closed Loop Referral technology solution that allows providers to connect residents to resources and services that address social determinants of health. The platform also allows providers the ability to more effectively track outcomes related to these referrals. This will result in greater efficiencies, better understanding of needs and gaps, and improved care for NH residents.

In this role, you will onboard and support community-based organizations within your designated region. This position demands strategic planning, particularly in cultivating buy-in from local stakeholders. You will be responsible for outreach efforts, hosting meetings, and building relationships with clinical providers, government agencies, and community organizations to establish trust through our proven approach to fostering strong collaborative communities. As a vital member of a small team working alongside the Unite Us community engagement team, effective project management and collaboration with colleagues are essential.

ESSENTIAL DUTIES & RESPONSIBILITIES:

Designated regions for Regional Engagement Coordinators are matched to New Hampshire Regional Public Health Network catchment areas. We are seeking two dedicated professionals to fill Regional Engagement Coordinator roles in different catchment areas. Each position will be assigned to one of the following catchment areas:

Catchment Area 1: Carroll County, Central, Seacoast, and Strafford County Public Health Networks of NH
Catchment Area 2: Capital Area, Winnipesaukee, Greater Manchester, and South Central Public Health Networks of NH

  • Onboard strategic priority partners alongside the Unite Us team and community providers to create and/or enhance the NH Care Coordination Network.
  • Work closely with the Unite Us team to analyze network data to provide recommendations for network growth and adoption (i.e., increasing the number of active organizations in the network or the number of active users).
  • Identify key organizations and influencers across core service domains to engage in the NH Care Coordination Network.
  • Conduct provider interest meetings and maintain a pulse of community interest and engagement.
  • Act as a partner advocate to drive the adoption of Unite Us within network organizations and their community partners.
  • Work with the Unite Us team to plan, host, promote, and/or present quarterly virtual Community Conversations targeting 211 organizations to grow network participation and increase utilization of the 211 Coordination Center partnership.
  • Leverage existing communication channels and create new channels, as appropriate, to support community engagement and network growth goals.
  • Work in partnership with the Unite Us team to Develop and run the Provider User Group and the Closed Loop Referral Network Participating Provider Engagement Program (NPPEP).
  • Work collaboratively with formal and informal partners and stakeholders across multiple and diverse community sectors, including nonprofits, health, government, schools, and business.
  • Participate in education, training, and technical assistance opportunities.


EDUCATION, EXPERIENCE, AND SKILLS:

  • Excellent communication and presentation skills – you’re an active listener who can build relationships, break down complex ideas, and tailor your message to a specific audience
  • A natural relationship builder- you empathize with others and quickly build social capital
  • Empathy and business acumen - you build trust with partners by understanding their motivations and bigger-picture business objectives and connecting those to the NH Care Connections network and mission
  • Comfortable engaging with end-users, promoting the NH Care Connections Network, and generating buy-in across multiple stakeholder groups.
  • You are solutions-oriented and highly adaptable
  • A passion for healthcare or human services
  • Typically requires a bachelor's degree or equivalent experience
  • Willingness to travel within the assigned public health region(s).
  • Valid driver’s license, reliable transportation, and proof of insurance
  • Commitment to working intentionally on diversity, equity, and inclusion across all responsibilities
  • Strong ability to develop and maintain strong, trusted, collaborative relationships with internal and external stakeholders and colleagues
  • Knowledge of federal, state, and local government, nonprofit and public health systems
  • Considerable experience managing multiple priorities with a high degree of flexibility
  • Proficient in Microsoft Office, databases, and web-based applications (internet, email, social media)

ENVIRONMENT:

The venue of this position is a general office environment that is clean, well-lighted, and environmentally comfortable, free of fumes or airborne particles, and only minimally exposes employees to office solvents limited to those relative to copier equipment.

This position requires:

  • Minimal requirement to reach at or above shoulder level; occasional reaching below shoulder level required
  • Ability to travel throughout the territory required
  • Ability to work a flexible schedule, which may include night and weekends
  • Ability to tolerate prolonged sitting and/or standing and to lift to approximately 25 pounds to waist high level whenever necessary. Mobility includes regular sitting, some standing and walking. Sitting, some standing and walking.
  • This position includes up to 50% travel within the assigned region(s)
