About the Job

Description

Greater Nashua Mental Health is looking for a full-time Coordinator of Community Based Services to join our Adult Services Team! The Coordinator of Community Based Services administratively manages a team within the Community Support Services Department. The team consists of Case Managers and Community Support Specialists. The Coordinator of Community Based Services is responsible for the Team’s performance on KPI’s, including productivity expectations and paperwork completion. The Coordinator of Community Based Services oversees the professional development of direct reports and assists in ensuring that staff are provided adequate supervision and support with the ultimate goal being that clients are empowered to lead full and satisfying lives. In addition to managing team, provides direct support, education, and outreach to adult clients diagnosed with severe mental illness.

Duties & Responsibilities: 

  • Provides direct client services for at least 10 hours per week. 
  • Assists consumers of mental health services with linkage to needed resources outside of the mental health center, including but not limited to medical, educational, and social services.
  • Completes all treatment plans, Adult Needs and Strength Assessments (ANSA), and Case Management Care Plans (CMCP) by due date listed in EMR. 
  • Completes all Quarterly Reviews within a two-week window of the end date of the quarter being reviewed.  Completes all other documentation within 3 business days (with first day being date of service/completion).
  • Reviews staff documentation (Tx plans, ANSAs, CMCPs, Quarterlies, etc.), as necessary.
  • Provides feedback to facilitate the necessary learning for staff to be effective, as necessary.
  • Monitors team KPIs and meeting notes and manages CM OpenAccess Holds.  Provides the necessary support to her/his teams to ensure that the team is meeting or exceeding performance objectives.
  • Provides weekly, one-hour, individual supervision to direct reports and documents the content of the supervision sessions.   
  • Prepares for and attends meetings, including team meetings (1x/week), 7 Prospect leadership meetings (weekly), CCBS Group Supervision (weekly). community-based staff members' meetings (monthly), department meetings (bi-monthly), etc and others as needed
  • Supports teams in navigating difficult times and challenges.  Checks in on teams and individuals, and models positive and professional behaviors.  Assists in organizing team building activities.
  • Performs other duties or special projects as required or as assigned.

Qualifications: 

  • Bachelor’s Degree in Psychology, Social Work or related field.
  • 3-5 years of work experience
  • Ability to teach effectively.
  • Familiarity with community resources.
  • Strong collaborating skills and an ability to work effectively both independently and as part of a team.
  • Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive.
  • Must possess a valid driver’s license and automobile with liability insurance in the amount required by agency policy
