As the State-designated Community Mental Health Center for Southern Hillsborough County, we serve everyone who walks in our doors, and no one is ever turned away due to the inability to pay for services. Without our agency, many of the most vulnerable members of our community would not have access to critically needed behavioral health care. About the Job
Greater Nashua Mental Health is looking for a full-time Clinician to join our Emergency Services Team! The Clinician works with a team of professionals — including first responders, healthcare providers, and community-based agencies — to stabilize a psychiatric crisis, avoid hospitalization and/or incarceration, and initiate immediate community-based treatment. The Clinician performs evaluations both in office and in the community including homes, hospitals, community and mobile-based milieus. The Clinician provides therapeutic crisis intervention and stabilization to individuals/families in need, to prevent or reduce potential suicide, serious debilitation, violent behavior and decrease the need for hospitalization. The Clinician applies differential diagnostic skills, provides telephone triage, mental status assessments, communicates with collateral contacts with family supports, extended evaluations, resource screenings, referrals for psychiatric consults, and assessments for appropriate disposition.
Duties & Responsibilities:
- Provides emergency crisis evaluations and all related service delivery requirements for youth, adult and geriatric populations in multiple settings, including hospitals, community-based settings in the region and/or on-site at GNMH locations.
- Utilizes a variety of Evidenced Based Practices (EBPs) and screening tools for risk assessment and crisis stabilization efforts.
- Formulates and initiates plan of action independently and/or in consultation with the client, Psychiatrist, Case Manager, and various other staff, as appropriate.
- Evaluates, conducts treatment planning, and provides support to internal GNMH programs, as it relates to crisis consultation and intervention.
- Provides assessment, treatment interventions and short-term crisis counseling and case management.
- Assesses the client's crisis state, immediate needs, and level of urgency, and selects the appropriate mode of intervention in relation to suicide risk, violence potential, current level of coping, mental status, support system, need for medication, crisis housing, brief hospitalization, IEA, or referral for outpatient treatment.
- Manages the acute phase of clients presenting in behavioral health crisis; assess, triage and refer clients to the appropriate services.
- Develops and maintain collaborative working relationships with hospital-based staff and other providers in order to effectively serve all clients in crisis.
- Maintains current working knowledge and use of all related, internal and external resources, policies, procedures, regulatory and compliance requirements necessary to effectively serve clients with quality care in multiple settings and with ever changing program needs.
- Answers telephone as needed and provides timely, appropriate assessment and determines next steps in support for clients with behavioral health needs.
- Willingness to transport clients as needed.
Qualifications:
- Master’s Degree in Clinical Mental Health Counseling, Social Work or related field.
- 1-2 years of related experience.
- Ability to form rapid rapport and interact with a broad range of clients, including severe and chronic populations.
- Strong collaborating skills and an ability to work effectively both independently and as part of a team.
