About the Job

Description

Greater Nashua Mental Health is looking for a full-time Case Manager to join our Substance Use Disorder (SUD) Services team! The Case Manager delivers high quality outreach and case management services to adults and adolescents in outpatient substance use disorder treatment at GNMH to support client’s engagement in treatment and ongoing recovery.  This position also delivers contingency management services to eligible individuals within the program.  Services are primarily provided within an office-setting, with some community-based outreach required.  Flexible schedule includes some early evening hours.

Duties & Responsibilities: 

  • Promotes successful treatment initiation and engagement through outreach and collaboration with referral sources for “warm handoffs”.
  • Works collaboratively with clients to assess service needs and develop plans that maximize client choice and control.
  • Links and effectively coordinates care with resources outside the mental health center, including health services, educational or vocational programming, social services, etc.
  • Coordinates referrals, entries, and discharges to higher levels to care to support continued engagement in services and progress toward Treatment Plan Goals.
  • Educates and enrolls eligible clients into the Department’s Contingency Management Program and delivers contingency management services in accordance with established protocols. 
  • Works as part of an multidisciplinary team and communicates and collaborates with colleagues, collateral contacts, supervisors, etc. regarding client care. 
  • Coordinates drug testing for clients, including unsupervised collection of samples, and communicates results with clients and team members. Consults with the laboratory to interpret results as necessary.
  • Supports clinical services by providing program orientation, harm reduction education, etc. to clients as directed.
  • Completes documentation requirements in keeping with GNMH, BDAS, and DHHS requirements.
  • Ensures service delivery requirements are met.
  • Manages and responds to emails, voicemail, etc. in a timely and professional manner.
  • Performs other duties or special projects as required or as assigned.

Qualifications: 

  • Bachelor’s degree in Psychology, Social Work, Human Services or similar field
  • 1 to 2 years of work experience
  • Preferred Certified Recovery Support Worker (CRSW) or willingness to actively work towards CRSW Certification while in the position. 
  • Experience working with people with substance uses disorders preferred. 
  • Familiarity with community resources.
  • Ability to work with diverse populations, including adolescents. 
  • Excellent communications and interpersonal skills
  • Excellent organizational and time management skills 
  • Knowledge of Microsoft Office products (Email, Word, EXCEL). 
  • Strong collaborating skills, and an ability to work effectively both independently and as part of a team.
  • Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive.
  • Ability to multitask and be flexible with regard to workload and assignments.
  • Ability to exercise reasonable judgment that bests supports GNHMC mission and policies.
