Lakes Region Mental Health Center (LRMHC) is the community mental health center serving Belknap and southern Grafton Counties. We provide services to 24 surrounding towns, and nearly 4,000 patients each year. A non-profit, private corporation founded in 1966, Lakes Region Mental Health Center is governed by a volunteer Board of Directors dedicated to our mission.  We provide integrated mental and physical health care to children, adults, elders and their families who are living with – and recovering from- mental illness and/or emotional distress. Our dedicated professionals provide the highest quality of healthcare and collaborate with many other organizations in the region to improve access to care and meet the mental health needs of the community. This is a great opportunity to work with our Assertive Community Treatment team. This position shares on-call responsibility with other members of the team. Rotating nights and weekends are included.

As a Nurse on our ACT Team you will be providing service coordination for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team as well as consulting with community agencies and families to maintain coordination in the treatment process. You will document client progress to maintain records and under the direction of the team psychiatrist you will supervise psychopharmacologic and medical treatment per protocol and makes appropriate referrals to community physicians for further assessment and treatment if indicated. Demonstrating skill and competence in establishing supportive trusting relationships with clients is essential

Qualifications: Licensure as an RN, RNC or LPN in the state of NH required. One to three years’ experience in health care with emphasis in mental health preferred. Special certifications in psychiatric nursing preferred.
