About the Organization

Community Crossroads is a 50(c)(3) not for profit corporation serving  individuals with disabilities and/or chronic health conditions and their families from birth to elder years. We have been in existence since 1979 serving eleven towns in southern New Hampshire but have recently expanded some of our services throughout the state. We are a conflict free case management agency and subcontract all direct services to 22 other non -profit and for- profit corporations.

We are designated by the state of New Hampshire as an Area Agency for developmental services. As such, we are responsible for service coordination; family support ; Medicaid billing; and quality assurance to individuals with developmental disabilities or acquired brain disorders. In addition, we are a licensed case management agency under the Choices for Independence program serving elders and other adults with chronic health conditions. We are also a Partners in Health site serving children and adolescents with chronic health conditions. Ancillary programs include Representative Payee services and Oral Health coordination. In total, we serve approximately 1,600 individuals and their families.

Our offices are located in beautiful Atkinson, New Hampshire, just over the Massachusetts border in close proximity to Interstates 93 and 495.

WHAT WILL YOU DO?

We are looking to add to our professional team of Service Coordinators. In this role, through a balance of direct contact and documentation requirements, you will ensure the people we serve receive the quality services they need to be successful in their lives and in their communities. Ideal candidates will demonstrate excellent verbal and written communication skills, be highly organized, have an optimistic attitude, and a desire for both interpersonal contact and utilizing online databases and electronic record systems. The candidate must also demonstrate a commitment to the important work we do!

RESPONSIBILIITES

  • Assist individuals and families to advocate for themselves and advocate on behalf of individuals and/or family for services.
  • Provide direct assistance to individuals/families as needed and as required by agency and state regulations.
  • Facilitate service planning process and team meetings to assure meetings promote individual choice & control and address the needs of the individual/family.
  • Develop a trusting relationship with individuals and families to foster open communication.
  • Facilitate and develop annual service agreements and amendments for all individuals with community care waiver funded services.
  • Monitor implementation of funded services, progress, and addressing any needed changes.
  • Assure individuals have knowledge of application process for benefit entitlements which are necessary for funding and to assure that all annual Medicaid re-determinations are completed as required.
  • Assess the individuals need for services and complete necessary requirements to assure the person is identified as in need of services in electronic record systems.
  • Complete all required documentation and maintain an electronic record systems.
  • Assist families in the development of services. This includes assisting in the selection of a service provider, that will meet the needs identified in Individual Service Agreement.
  • Monitor services to ensure the health, safety, and basic needs of the individual are being met and to report any allegation of abuse, neglect, or exploitation as required by agency policy and state regulations.
  • Work collaboratively within the agency departments as well as with the Bureau of Developmental Services—ensuring that timely and effective communication occurs related to the individual’s support needs
  • Attend necessary statewide and agency wide meetings and trainings related to the roles and responsibilities of a service coordinator
  • Represent the agency in a professional manner at all state and local meetings.
  • Ensure continuity and quality of services.

QUALIFICATIONS

  • Two years’ experience working in human services with individuals or families or related field or Associate/Bachelors Degree in Psychology or Social Work field.
  • An understanding and respect for the needs of individuals with chronic medical needs, an understanding of advocacy, self-advocacy, concepts of self-determination and person-centered services.
  • Strong skills in the areas of communication, problem solving, conflict resolution, facilitation, advocacy, group process and mediation coordination and monitoring of services.
  • Ability to empower individuals through modeling, encouragement, providing accurate information and offering non-judgmental support.
