Position: Intake and Eligibility Specialist

Location:  10 Ferry Street Concord NH 03301

FLSA Status: Nonexempt, Part Time 

Reports to:  Executive Director

 

Position Summary: 

The Intake and Eligibility Specialist is responsible for reviewing relevant documentation to make recommendations regarding eligibility for developmental disability and acquired brain disorder services. Reviews application materials including standardized assessment reports (SIB-R, Vineland, ICAP, etc.), medical reports, and other relevant reports to make Tier I eligibility recommendations to Area Agencies. Participates in related meetings, data collection, and projects as required. The tasks required for this position can be completed remotely.

 

Essential Duties:

  1. Receive and review documents submitted by participating Area Agencies to recommend:
    1. A finding of eligible, conditionally eligible, or ineligible for services under He-M 503 
    2. A finding of eligible or ineligible for non-waiver services under He-M 522
    3. Referral to a Tier 2 review for clinical consultation.
  2. Within 5 working days of receipt of documents, provide the appropriate Area Agency with:
    1. Recommendation regarding the determination of eligibility 
    2. Recommendation to obtain additional documentation 
    3. Referral to Tier 2 consultant
  3. Advise Area Agencies of recommendations made by Tier 2 consultant
  4. Facilitate the Statewide Intake Coordinators group.
  5. Be available to regions for consultation and training regarding intake, eligibility, SIB- R, and compliance with regulations as requested through CSNI.
  6. Participate in data collection projects and tasks related to intake and eligibility tracking and outcomes.
  7. Other duties as requested.

 

Experience, Skills, and Educational Requirements:

  1. Minimum of bachelor’s degree in social work, Human Services, Psychology, Education, or another related field, and,
  2. Minimum of 3 years of experience working with people with an intellectual or acquired brain disorder. Knowledge of standardized educational and vocational evaluations is a plus.
  3. Able to independently utilize Microsoft Word and Excel, cloud-based storage platforms including ShareFile, and other internet-based software products in a way that adheres to HIPAA regulations.
  4. Ability to construct professional communications and understand common clinical documents.
  5. Proficiency in assessing and applying information and objective data to a set of established criteria for determination of applicability and eligibility.
  6. Ability to maintain confidentiality and maintain HIPAA compliance in all communications, verbal, electronic, or written. Ability to multitask, meet deadlines, and deliver high-quality and accurate results.
  7. Excellent verbal and written communication skills.
  8. Ability to speak with confidence and clarity and make professional presentations to a varied group of stakeholders. 
  9. Excellent customer service skills and interpersonal style. 
  10. Positive, professional attitude and overall demeanor.

  6. Simultaneous handling of multiple tasks frequently. 
  7. Occasional travel required.
