AIDS Response Seacoast (ARS) is a non-profit community-based HIV/AIDS Service Organization, based in Portsmouth, NH, dedicated to providing education, direct assistance and advocacy to persons and communities affected by HIV infection. We provide direct service to clients at all stages of HIV infection and educational programs throughout Seacoast New England. ARS evolved as a grassroots response to the epidemic in 1987 to offer vital emotional and practical support to people living with HIV infection and AIDS. Services provided by ARS are free of charge to persons living with HIV.

Position Summary:
Reporting to the Executive Director/Ryan White Program manager, the Medical Case Manager (MCM) provides medical and social services case management to adult clients with HIV/AIDS in the Seacoast Communities. This role provides high-level advocacy for client’s clinical, psychosocial and social determinant of health needs (SDoH). The MCM provides comprehensive case management services, manages biannual applications for state medication assistance programs, as well as assistance with other state, federal and private benefit programs and insurance. MCMs work in coordination with state programs, and provide coordination and advocacy with other medical and psycho-social providers to provide optimum care for clients and their ongoing and emerging health needs.
Requirements:
- Bachelors or Associates degree or minimum of 2-years professional experience in social work, human services or a related field.
- Knowledge and experience with HIV services, including HIV education/prevention, outreach, counseling, emotional/psychological support and/or case management preferred.
- Knowledge of federal and state benefit programs and experience advocating for clients.
- Awareness of substance abuse and mental health issues.
- Ability to work within a wide community and social service network.
- Proven ability to work with clients from culturally, ethnically and racially diverse backgrounds.
- Excellent written and verbal communication skills.
Responsibilities:
- Provides case management services to HIV positive clients. Conduct intake assessment, reassessment to determine medical, mental health, financial, housing, legal, preventative and other needs of each client.
- Provide advocacy and referrals to services to meet those needs, including coordination of federal, state, local and private assistance programs and assistance with accessing/applying for benefits.
- Reinforces self-determination and utilizes client teaching opportunities to ensure medication adherence and client engagement.
- Work with clients to develop and implement treatment plans that include long and short-term goals, and defined activities to achieve those goals.
- Collaboratively works with medical and other service providers to coordinate care.
- Attends team meetings as needed and as requested by team leaders
- Completes all paperwork and reporting within the required time frame
- Maintains and manages the ARS food pantry which may include shopping, picking up food donations and assisting clients in food pantry.
