Medical Social Worker (Home Care)                                                                     

Concord Regional VNA is seeking an experienced Medical Social Worker to provide counseling and support of patients and families in the Home Care program. Communicate to patients and families about resources to meet their needs. Work collaboratively with a multi-disciplinary team of Home Care professionals to provide quality care, support and education to the patient and their families. Must be well organized, have good communication skills and have strong time management and multi-tasking abilities.


Master of social work degree (MSW) from a school of social work accredited by the Council on Social Work Education.  Two years of Social Work experience in the healthcare setting. ACSW Credential from the National Association of Social Workers and previous Home Care experience desired.

Work Schedule:  

Per Diem, no weekends or holidays required. Very flexible schedule.
