Community Health Worker (CHW) Recovery Services

Community Health Workers will primarily be working out in the community with specific target populations. CHW's will work closely with behavioral health, substance misuse and medical providers, primary care teams and other agencies to improve patient care and outcomes.
 

RESPONSIBILITIES & AUTHORITIES:

1)        DATA COLLECTION:
a.       Responsible for GPRA Patient Data Collection that includes collecting information from the patient at intake, 6 month follow-up and 12 month follow-up timeframe.
b.       Works with integrated MAT team to assist with addressing vulnerabilities in social determinants of health.
c.        Reaches out to patient to assess and address barriers in collecting data
2)       EDUCATION ACTIVITIES:
a.       Works with MAT team, supervisor, and interpreters to identify language and low-literacy materials to use with patients to provide information and resources to better manage their health condition(s).
b.       Assess potential resources to provide education to patients such as: Pamphlets, videos, posters, classes, one on one education and share with MAT Team for consensus and planning.
c.        Assist with preparing MAT intake packets as well as setting up information sessions for the public
3)       COMMUNICATION:
a.       Works directly with patient to ensure collaboration of healthcare and community service support providers addressing barriers with patient accessing needed social services.
b.       Documents interventions directly in patient’s EMR (Electronic Medical Record). Document activities, service plans and results in an effective manner while strictly adhering to the policies and procedures in place
4)       CARE COORDINATION:
        a.    Facilitate communication and coordinate services between providers
        b.    Utilize resources such as MCHC Case Managers, other MCHC Ancillary support staff and collaborating community agencies to assist with care coordination
5)       OUTREACH:
a.       Coordinate patient drop-in information presentations by MAT Team member(s).
b.       Assist with expanding access by developing/maintaining relationships with collaborating and referring organizations and maintaining supply of MCHC MAT Program information flyers ‘par levels’ with these community organization.
6)       OTHER:
a.       Establish positive, supportive relationships with patients, family, and visitors.
b.       Maintain positive, collaborative relationships with other community service organizations and referring organizations.
c.        Motivate patients to be active, engaged participants in their health
d.       Effectively work with people (staff, clients, doctors, agencies, etc) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions
e.       Build and maintain positive working relationships with the patients, providers, nurse case managers, agency representatives, supervisors and office staff
f.         Continuously expand knowledge and understanding of community resources, services and programs provided
g.       Other duties as assigned
