Clinical Coordinator - ACT Team

Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a full-time Assertive Community Treatment Team Leader to join our talented multi-disciplinary team in our Community Support Programworking with adults living with severe and persistent mental illness. 

Assertive Community Treatment (ACT) is an evidence-based practice utilizing a multi-disciplinary team based approach designed to support people with severe mental illness who are most at-risk of psychiatric crisis, inpatient hospitalization, homelessness, substance use disorders and involvement in the criminal justice system.  Providing consistent, caring, person-centered services in the community allows for an increased likelihood of positive outcomes and improved quality of life for ACT clients. 

As Riverbend’s Assertive Community Treatment Team Leader, you will be responsible for the operation of your team, including the provision of clinical care, staff supervision, fiscal accountability, risk management, and participation in short and long-range planning of services to meet the needs of the community.

As Riverbend’s Assertive Community Treatment Team Leader, you will be responsible for the operation of your team, including the provision of clinical care, staff supervision, fiscal accountability, risk management, and participation in short and long-range planning of services to meet the needs of the community.

Requirements:

• Master’s degree in Psychology, Social Work, Education, Rehabilitation or Human Services. 
• Evening and weekend availability
• Excellent communication, problem-solving, time management, and organizational skills
• A valid driver’s license, personal vehicle, and auto insurance.
• Previous supervisory experience preferred

Responsibilities:

• Supervising a team of Bachelor’s and Master’s level case managers and clinicians who support clients with severe and persistent mental illnesses to achieve optimal recovery and independence
• Ensuring adherence to the Assertive Community Treatment model, as well as other evidence based practices
• Facilitating daily operations of the ACT Team
• Maintaining a caseload of adults with severe and persistent mental illness.
• Coordinating with various outside agencies, guardians, and other treatment providers
