Children’s Clinical Case Manager

Summary Statement:

The Clinical Case Manager provides case management services as needed, completion of the CANS assessment to children and families, referral and coordination between outside agencies and functional support services as deemed medically necessary among the team.  This is a full time, 35 hour per week position.  


Qualifications and Skills:

  • Master’s degree required;
  • Experience within a clinical setting providing community supports;
  • Willingness and ability to work in homes, school settings or other community settings;
  • Must be able to flex schedule to accommodate needs of clients and be responsive to office space issues of agency;
  • Effective communication skills, organizational skills, and ability to multi-task;
  • Strong documentation skills and prior experience utilizing electronic records;
  • Strong ability and desire to work as part of a clinical team.

Physical Demands and Working Conditions:

  • Must have ability to sit at computer and use keyboard for data entry;
  • Must be able to flex schedule to accommodate needs of clients and be responsive to office space issues of agency;
  • Must have a clean driving record, reliable transportation, ability to travel within the community on a daily basis and proof of adequate auto insurance;
  • Must have ability to sit for 50 minute intervals.

Major Functional Areas and Description of Duties:                          

  • Conduct family-centered interviews with the clients and family members to assess support needs;
  • Assess for and make referrals to functional support services or other community supportive services;
  • Provide timely documentation on each service delivered;
  • Collaborate and coordinate efforts with clients’ treatment team and other involved providers;
  • Develop cooperative working relationships with all the agencies serving the client and their family members;
  • Provide FSS FTC to families, complete eligibilities and develop treatment plans with each client; specify presenting problem, objectives, interventions, and desired outcomes;
  • Adhere to required reporting, and confidentiality standards as required by state laws and wraparound standards;
  • Establish a trusting relationship with the child/youth client and his/her family.
