Homeless Intervention Specialist

Tri-County CAP is dedicated to improving the lives and well-being of New Hampshire's people and communities. TCCAP provides opportunities and support for people to learn and grow in self-sufficiency, and to get involved in helping their neighbors and improving the conditions in their communities.  TCCAP connects people throughout Coös, Carroll and Grafton Counties with resources that can help them with home energy costs and conservation, emergency intervention, homelessness and hunger, domestic violence and sexual assault, restorative justice issues, visitation and family counseling, sobriety, transportation, Head Start services and dental care, as well as guardianship services to New Hampshire residents statewide.

Tri-County CAP is a multi-program non-profit agency that is dedicated to improving the lives and well-being of New Hampshire’s people and communities.  The goal of our Homeless Intervention/Prevention program is to alleviate immediate housing emergencies and to assist shelter residents with achieving independence that leads to permanent housing and ends the cycle of homelessness.

The Homeless Intervention Specialist position will perform outreach to the homeless within the Upper Valley (Lebanon) and Northern (Littleton) regions of Grafton County in an effort to assist and engage those who are homeless towards finding permanent housing as well as treatment for mental illness and/or substance abuse.  These are full time, 40 hour a week positions.


  • Provide support in obtaining essential services within the community such as food stamps, mental health referrals, Medicaid, transportation, and disability services.
  • Serve as an advocate for members by acting as a liaison between them and community programs, and social services. .
  • Conduct HMIS assessments.
  • Provide unsheltered households with support in obtaining basics such as housing, healthcare, and education.
  • Create and implement outreach strategies to ensure the success of the program.
  • Serve as an emotional support system by providing engagement and case management to the unsheltered in the community.
  • Determine type and availability of resources needed to meet individual needs of members.
  • Utilize coordinated entry system as well as mainstream resources to obtain permanent housing whenever possible.
  • Obtain SOAR certification to assist with obtaining disability and benefits for those who are unsheltered and qualify.
  • Attend scheduled monthly and quarterly Coordinated Entry/Homeless Providers/PATH meetings and other meetings to stay up to date on current trends and regulations as requested by supervisor;
  • Work closely with various local town and city welfare offices and human services agencies as needed.

Qualifications, Skills and Experience:

  • High level of interpersonal skills to handle client services situations with tact.
  • Ability to problem solve and take action as appropriate.
  • Experience with crisis intervention
  • Knowledge of local and community mainstream resources.
  • Demonstrated ability to be self-directed and motivated with minimal supervision.
  • Knowledge of program services, client needs and management techniques, needs of client populations and treatment sources.
  • Understanding of homelessness and experience with mental illness, substance abuse disorders.
  • Skills in prevention and diversion in an effort to reduce homelessness.
  • Frequent travel is expected, valid Driver’s License a must


One to three years of human services preferred. Ideal candidate will have at least a two-year degree in human services or related field.
