Easter Seals Military & Veterans Services, in partnership with Veterans Count, responds rapidly, efficiently and effectively to the unmet needs of Service Members, Veterans, and their Families to ensure that they can thrive in their communities.

The Care Coordinator provides assessment, planning and service coordination to service members, veterans and their families. The primary focus of this position is in employment for Veteran's. Responsibilities will include receiving referrals of service members/families, responding promptly to new referrals and calls/requests from existing clients, completing comprehensive intake/assessment/care-planning process with service member and families and serve as "one stop" resource for coordinating services - assist in accessing services provided by military, civilian social service system, VA, Veterans Count, and other community resources. Work in collaboration with other service providers to maximize use of existing resources, facilitate prompt/comprehensive services, and minimize red tape and stigma for service members and their families. Participate in data collection/analysis activities to ensure effective program evaluation.

The hours for this position have not been determined yet. Full time positions offer benefits, including medical, dental, matching retirement, life insurance, potential for tuition reimbursement, as well as ongoing professional development with substantial room for growth within a nation wide human services agency.

Requirements:

A Masters in Social Services/Counseling or related field is preferred (Bachelors acceptable with related experience), as well as an understanding of military structure and culture. Experience in assisting others in finding jobs and working with employers is preferred. This position requires the ability to work as part of a team and a reliable vehicle and insurance which meets our coverage requirements.
