Functional Support Services Provider

Harbor Homes is continuing to grow!  Come join our team and help us with our efforts to assist individuals in need.  Be part of our new program supporting clients with severe and persistent mental illness successfully transition from hospital facilities to independently living in the community. 

As a Functional Support Service Provider, you will work with the Program Manager and interagency treatment teams to implement services prescribed in individual treatment plans.  You will contribute to the development of the treatment plan, provide recovery based functional support services and crisis intervention support as needed. 

Responsibilities to include facilitating referrals to groups, peer support resources and other recovery based activities; providing guidance with interpersonal skills development, behavioral modification, basic living skills to include budgeting, cooking, personal hygiene maintenance, self-care, housekeeping, independent use of public transportation and independently accessing community resources and services; conducting client assessments to evaluate satisfaction with quality of life, mental health status and functioning and substance abuse issues; and completing all documentation accurately, completely and within 24 hours of provided service.

The successful candidate must possess a Bachelor’s Degree in psychology, social work, rehabilitation, education or related human service and a minimum of one year experience working with individuals with mental illness.   The ability to work effectively with individuals, exercise good judgement and use current technologies and software such as electronic medical records also necessary.
