Housing Stability Case Manager

Cross Roads House

Cross Roads House provides shelter and services to homeless individuals and families in the Greater Seacoast area.  We provide both emergency shelter and a transitional program to help our residents move with dignity and purpose to stable and decent housing. www.crossroadshouse.org


 Cross Roads House seeks a Housing Stability Case Manager (HSCM) for an established community-based Housing First Permanent Supportive Housing program serving individuals who have been chronically homeless.   To promote housing stability, the HSCM provides individualized, intensive case management services, including housing search assistance and landlord/tenant support. Grant-funded, full-time, flexible hours. 

Requirements:

Ideal candidates will have at least 3 years of relevant experience in working with people experiencing homelessness, mental illness, substance use disorders and chronic medical conditions.  Excellent advocacy and collaboration skills are essential.    Master’s Degree preferred; equivalent education and experience considered.  

Responsibilities:
  • Work with partner agencies and other shelter staff to identify and engage potential clients for the project.
  • In the course of collaborative work with clients, assess for strengths and service needs, including benefits, employment, mental health, substance use, money management, social connection, and self-advocacy
  • Assist clients in obtaining housing, including explaining program to and negotiating leases with landlords,  apartment inspections, completing the participation agreements with clients, helping clients with apartment set-up and orientation to apartment living
  • Support positive landlord-tenant relationships, as needed, and work to prevent lease violations and evictions
  • Assist clients with re-location as needed
  • Establish client-directed Stability Plan goals, identifying appropriate steps towards achieving goals, modifying goals as needed, and tracking progress
  • Facilitate a wide variety of community resource referrals, coordinate with service providers, and advocate on behalf of clients as appropriate
  • Utilize de-escalation, motivational interviewing, harm reduction, and other clinical skills to maintain client engagement and address or prevent crisis situations
  • Provide telephone on-call support to project clients in cooperation with front desk staff and other members of the social work team
  • Maintain precise and accurate documentation of social work services, including paper and electronic records
  • Adhere to professional guidelines, including maintaining appropriate boundaries with clients and ensuring appropriate confidentiality
  • Collaborate with other staff members so that individual client needs are effectively addressed
  • Provide qualitative and quantitative information about client progress, needs, and program outcomes, including using the HMIS data collection system as necessary
  • Recommend and support improvements in program design and operations
  • Participate in local and region collaborative efforts and represent Cross Roads House as requested
