Case Manager

Building Community in New Hampshire

Building Community in New Hampshire (BCNH), formerly known as Bhutanese Community of New Hampshire, is a tax exempt 501 C (3) nonprofit organization established in 2009. The mission of BCNH is to be a multi-ethnic, Mutual Assistance Association that provides an array of services required by diverse refugee and immigrant communities to help newcomers understand and navigate the systems that are part of everyday life. BCNH is looking for a dynamic, hardworking and innovative part-time Case Manager who is interested in making a difference in people’s lives through active participation in BCNH activities. To offer post resettlement case management services to New Americans. This position requires strong knowledge of case management and refugee resettlement. Case Manager will be responsible for providing information, coordinating welfare benefits and other services to New Americans in the city of Concord, Manchester and Nashua. This will entail provision of practical support to ensure that they remain safe, able to access services such as housing benefits and educational opportunities to promote their well-being and independence. This position bearer is responsible to encourage and engage New Americans and support them to strengthen their capacity to participate in community life. The post-holder will offer information, suggestions, casework and advocacy and liaise with all appropriate services to make sure New Americans’ needs are appropriately assessed and met in a culturally and linguistically appropriate manner. The Case Manager will be reporting to the Program Director.

Requirements:

• Must possess excellent verbal and written communication in Swahili, French, Kinyarwanda and English languages.
• Must be able to work independently, analyze and make sound judgments in order to resolve problems and concerns in a timely manner.
• Ability to maintain professional and cooperative relations with our stakeholders and partners.
• Associate degree or above is preferred.
• Must be flexible with schedule to meet the needs of the program.
• Must have valid driver’s license, a clean driving record and reliable transportation.
• Minimum of one to two years of volunteer experience in the community is preferred.
• Excellent attention to details required.
• Analytical, proactive and creative skills required.
• Team player, self-starter, ability to work independently are encouraged.
• Resourceful, highly organized, and able to adapt quickly to changing priorities preferred.

Responsibilities:

• Conduct outreach and screening of potential clients, identified through referrals by community members and agencies.
• Act as a link to eligible clients by providing them with referrals to mainstream community agencies to meet needs such as: housing, physical health care, mental health care, employment, education, and financial support.
• Refer eligible youth to the youth engagement program.
• Liaise with other agencies and professionals to establish appropriate availability and accessibility of their services.
• Engage, support and liaise with other staffs in the organization to meet their needs.
• Refer clients to outside agencies as appropriate for specialist help with issues beyond the capacity of the organization.
• Liaise and network with other agencies and professionals working with New Americans.
• Maintain accurate and up-to-date records, files, case notes, correspondence etc. and to gather statistics to monitor and evaluate the service providing reports as required.
• Undertake training and self-development in order to develop skills and increase knowledge and understanding of seniors’ services.
• Provide culturally and linguistically appropriate post resettlement services and comprehensive case management services for New Americans.
• Develop individual case plans with particular attention to culture, language, and special circumstances.
• Provide information, education, referrals, outreach, advocacy, and support to ensure that each New Americans receive the services they require;
• Coordinate/maintain communication with key stakeholders
• Conduct home visits as mandated by the program.
• Participate in regular training and communication sessions with both on-site and Program Director.
• Perform all other duties assigned by supervisor.
