Community Liaison

Concord Regional Visiting Nurse Association

Concord Regional VNA  is a not-for-profit, community-based healthcare provider that provides home care, hospice and community support services to  41 communities in central New Hampshire.  Concord Regional VNA has strong ties with these communities and works collaboratively with Capital Region Healthcare and other providers to provide exceptional care to its patients.  Our mission is to improve the health of the people and communities we serve by managing illness and promoting wellness through all stages of life. Job Duties:

The Community Health Liaison works collaboratively with assigned referral sources in the community to ensure a smooth transition onto Concord Regional VNA services. In addition to completing new home care/hospice referrals, the Community Health Liaison will work with facility staff to provide education about the scope of services and coverage/eligibility requirements for patients. The Community Health Liaison is responsible for cultivating and developing relationships with all referral sources in assigned facilities as well as completing referrals for appropriate patients to Concord Regional VNA.

Qualifications: Minimum of an Associate’s degree plus 5 years of experience in related field. Bachelor’s degree in related field preferred. Minimum of  1-2 years of recent acute care, home health, hospice and/or skilled nursing facility experience. Experience in community health, home health, or hospice sales and marketing preferred.

Work Schedule and Requirements: Full-Time (40 hours per week) Monday - Friday; 8 a.m. - 5 p.m. Flexibility to work additional hours as necessary. Travel to assigned accounts is approximately 80% of time. Rotating coverage for minor holidays.
