Behavioral Health Coordinator

The Behavioral Health Coordinator is responsible to assess, plan, implement and evaluate the Agency's  behavioral health program.  

Job Duties

  • provides clinical support, education and oversight of the behavioral health team
  • cultivates relationships in the community to develop referral sources 
  • coordinates patient care


  • Registered Nurse or Medical Social Worker with a minimum of 2 years of experience with casework in a medical setting. 
    • Registered Nurse will need to possess a current nursing license issued by the State of New Hampshire
    • Medical Social Worker will need to possess current LICSW license issued by the State of New Hampshire
  • Experience working with a behavioral health population preferred.
  • Experience in community health, home health, acute or skilled long term care setting is helpful.
  • Able to satisfactorily complete the agency’s RN clinical skills/competency test (including IV as required)
  • Ability to read and interpret written documents
  • Computer competency including the ability to retrieve information necessary to complete job responsibilities
  • Excellent time management and organizational skills (ability to prioritize daily work in a prescribed order that promotes efficiency, timeliness and accuracy)
  • Excellent verbal and written communication skills
  • Excellent phone etiquette
  • Excellent customer service skills (friendly, courteous and helpful)
  • Able to handle multiple tasks at one time
  • Able to understand and carry out directions and instructions
  • Able to work with minimum supervision
  • Able to read, write and speak English
  • Is physically and emotionally capable of performing the job requirements

   Work Schedule: 20 hours per week with pro-rated benefits package.
