Case Manager - Adult

For 40 years, West Central has provided community based mental health care provider for New Hampshire's Sullivan County and the Upper Valley. 

West Central just promoted a member of our CORE Team and now we need a replacement.

West Central Behavioral Health is the nonprofit community mental health care provider for Sullivan County and the Upper Valley, ensuring access to advanced counseling, treatment, and support for people of all ages, regardless of their ability to pay.

By joining our established team of dedicated professionals, you will be helping adults living with mental illness, by providing services in the community and office-based treatment through case management, skills training, psycho-education and professional support focused o assisting Consumers in working toward their practices throughout the Grafton County.


•Bachelor’s Degree (will consider near-graduates)
•Excellent driving record
•Reliable and insured vehicle
•Valid driver’s license
•Case management experience helpful
•The knowledge of psychology

•Provide case management and related services to an assigned case load
•Assist clients in managing daily problems and stresses, developing a new life structure via work or other meaningful activities, addressing and developing life and illness management skills, and providing family education and support
•Coordinate services with other agencies, application and management of benefits, developing and implementing a treatment plan reflecting an outcome orientated and function based philosophy.
•Work as a team member
•Maintain all documentation that is required in a complete and timely manner
•Provide all clinical services in ways that are outcome orientated, recovery based philosophy that is both effective and efficient.
•Provide direct service as 50% of your work each month
•Provide crisis interventions as required
•Attend and participate in staff supervision/training and attend clinical case presentation to assist in treatment planning and problem solving
