CTI Case Manager

Integrated Delivery Network

The IDN Care Transitions Project employs the evidence-based ‘‘Critical Time Intervention’’ (CTI) in which a multidisciplinary team provides care at staged levels of intensity to patients with serious mental illness during transitions from the hospital or shelter setting to the community. The Case Manager in the CTI team has responsibility to build supportive and strengths-based relationships with persons being served through CTI phases, including assessment, referrals, follow-ups and transitions. The CTI-CM assures continuity among prioritized goals in the care plan, seeking to remove barriers, access and coordinate services, and cultivate patient self-efficacy and personal support networks.

Requirements:

Bachelor’s degree in a behavioral science field from an accredited college/university, or combined education and experience; One year documented work experience supporting adults with serious mental illness and/or substance use disorder; Excellent communication, organizational, planning and time management skills; Demonstration of proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) with the ability to learn new applications; Knowledge of Strafford and Rockingham County communities is preferred.

Responsibilities:

Essential Job Functions:

Provides case management for an assigned group of patients in CTI; Assumes primary responsibility for developing, writing, implementing, evaluating, and revising overall goals in conjunction with the person-centered care plans.; Provides individual skill building and support network to promote independent living and wellness; Ensure that those who need mainstream benefit programs (e.g., SSI/SSDI, food stamps, Medicaid, etc.) are assisted in navigating the appropriate resources to obtain these benefits; Maintains on-going files, including care plans, correspondence and case notes; Must be willing to work in a position which is mostly community based, must be willing to provide services in community locations (i.e. homeless camps, shelters, jails, consumers residences, parks, emergency departments, etc.); Must have reliable transportation, maintain valid NH driver’s license; The use of personal vehicle with mileage reimbursement is required for this position. Must be able to pass a background check.
