Critical Time Intervention (CTI) Team Leader

The Critical Time Intervention (CTI) Team Leader provides services to clients and supervises the CTI Specialists. This position oversees all CTI services to assure a high quality of service delivery and commitment to the Greater Nashua Mental Health Center’s (GNMHC) Mission & Values.  This position is fulltime and is half clinical and half supervisory.

CTI is a structured program that provides supports to people who are transitioning from a hospital Emergency Department or in-patient hospital stay to needed community based services. The CTI staff must be flexible and creative as well as comfortable working primarily in the community. The program’s primary initiative is to locate and connect participants to adequate, affordable housing units as necessary, to connect them with appropriate mental health services, to collaborate with community providers on behalf of the participants, and to develop natural support systems.

The goal of the program is to assist consumers in making critical transitions in their lives; assist consumers in creating linkages to long term community supports; assess and evaluate consumer strengths and needs; work with other community providers in a team approach; work in the consumer’s community; mediate and advocate on behalf of consumers to help them obtain and maintain housing, health care, benefits, entitlements, and transportation; and monitor, record & track service delivery.

A Master’s Degree in Psychology, Social Work, Human Services or other related field is required.  Candidate should have at least two years of experience working with the population served. Experience supervising staff who are working with children, adults and families in a direct care setting is preferred.
