Central New Hampshire VNA & Hospice

The Mission of Central New Hampshire VNA & Hospice is “Promoting dignity, independence, and well-being through the delivery of quality home health, hospice and community-based care services”.

Central New Hampshire VNA & Hospice offers healthcare services in the homes of clients in Lakes Region communities by providing nursing services in the home, rehabilitation services (physical therapy, occupational therapy and speech therapy), Pediatric Care (direct health care, education and support services for children and families) and a comprehensive, team-based Hospice program. 

Central New Hampshire VNA & Hospice is a not-for-profit, Medicare-certified provider of home care and hospice services, licensed by the State of New Hampshire. The agency is governed by a volunteer Board of Trustees and supported by private and corporate donations. 

Responsible for the delivery of MSW services for the organization in compliance with agency policies.  Assists and counsels clients and families with financial, social and emotional concerns.  Provides participation in community provider pre-discharge patient assessment as requested.  Provides participation in service area projects or meetings as requested by supervisor or senior management.  Provides social work services in accordance with the physician’s plan of treatment.  Assists the physician and other members of the health care team to understand the significance of social and emotional factors related to the client’s care.  Assesses social and emotional factors in order to help the home care client and family to cope with problems in daily living and to help them follow medical recommendations to maximize the client’s health condition.

  1. Masters Degree in Social Work from a college/university accredited by the Council on Social Work Education and at least one year of social work experience in a health care setting
  2. Minimum of one year of experience in social work, preferably in a home care agency or  hospital, outpatient clinic, rehabilitation center or mental health program.
  3. Knowledge and Abilities:
    • Demonstrated knowledge and skills necessary to provide care primarily to the geriatric population, and to a lesser degree the pediatric and adult populations.
    • Demonstrated knowledge of the principles of growth and development over the life span.
    • Able to assess data reflecting the client’s status and interpret the appropriate information needed to identify each client’s requirements relative to their age-specific needs.
  4. Never been convicted of patient abuse, neglect or misappropriation of funds or property.
