Seeds of Faith is the parent company for Lydia's House of Hope, a Transitional Services Training Program for homeless women and children. Seeds of Faith has been operating since 2002.


Work Hours:    Part - Time Monday & Friday: 9-4; Wednesday 9-3

Working directly with the Case Manager, this position performs selected activities to ensure that the case management for residents, training programs and household run smoothly. The Assistant Case Manager must be a well-grounded, self-directed person that is able to take initiative and directions while being a motivated person who can work successfully with people whose lives are impacted by poverty. They must be able to maintain healthy boundaries and calmly follow safety protocols in a fast-paced environment. The employee must be able to meet established goals for the position and be punctual with good attendance, while working closely with the Case Manager and Executive Director. Working as part of a professional and para-professional team and maintaining a positive attitude and approach is of utmost importance. Attention to accurate documentation and follow through with residents served is essential. While adherence to house policies and procedures is imperative, the Assistant Case Manager must also be able to be flexible and accept change. This is currently a part-time (20 hour a week) position that has the possibility to expand into a full time position for the right candidate.

Responsibilities:

  • Assist case manager working directly with a caseload of up to 10 mothers and their children to ensure completion of personal goals
  • Act as liaison between community resources, case manager, executive director and residents to ensure in-house programming is provided at the highest level possible
  • Working directly with the women regarding financial planning and budgeting
  • Residential duties including, but not limited to: room inspections, chore assignments, dinner assignments and grocery lists
  • Administrative duties including, but not limited to: typing, filing, assignments, scheduling, etc.
  • Assist Case Manager with screening residential applicants, updating the wait list, contacting new residents, beginning the intake process for new residents, etc.
  • Maintain communication between second shift, weekend and overnight staff
Requirements:
  • BS/BA in Social Services, Human Services or similar program of study
  • Minimum of 2 years’ experience within the Human Services/Social Work field required, experience with Case Management or Administrative experience preferred.
