Case Manager, Developmental Services

Join our dynamic and energetic team of professionals committed to providing high quality Case Management services to adults (21+) with developmental and/or intellectual disabilities and/or Traumatic Brain Injuries (TBI).

Requirements:

An Associate’s Degree in related field plus relevant field experience is required. A Bachelor’s Degree in related field plus at least three years’ experience in a human service industry or a Master’s Degree plus one year experience in a human service industry is preferred.

As local travel is a requirement of this position, candidate must have a valid driver’s license, clean driving record, proof of adequate vehicle liability coverage, and reliable transportation.

All candidates must pass a background check which includes criminal and driving records.

Responsibilities:

Case Managers are responsible for service coordination and assuring the continuity of services for each client on their caseload. They organize, facilitate and document service planning and negotiate, monitor and evaluate the provision of the individual’s services. This includes evaluating client needs, prioritizing the level of service coordination, ensuring that client’s needs are being met by all service providers, and documenting outcomes. Case Managers assist clients with accessing resources and maintaining the highest level of community integration. Clear and open communication with the client and an inter-disciplinary team is essential. Some of the most important skills successful Case Managers possess are: time management, autonomous decision making, ability to triage multiple high-need situations, interpersonal skills, and the ability to adapt to change.
