Psychiatric APRN

Riverbend Community Mental Health

Founded in 1963, Riverbend Community Mental Health is a private, nonprofit organization providing specialized behavioral health services in central New Hampshire. Core services include 24/7 emergency mobile crisis services; counseling services for children, adults, and families; and community support, vocational services, and residential programs for adults and seniors. The Consulting APRN is responsible for supporting behavioral health care provided by the Merrimack County House of Corrections (MCHOC) and Community Bridges.  This position will assist two of our community partners and help them to deliver the best possible quality of care to their clientele.
 
Essential Duties and Responsibilities: 
 

  • Provide regularly scheduled consultation to participating community partners.  Consultations will focus primarily on patients who are new to behavioral health treatment or who are not improving as expected.
  • Work with participating partners to track and oversee patient outcomes using an integrated health model.
  • Suggest treatment plan changes including medication recommendations for patients.
  • Discuss patients who may need referral for additional behavioral health care and advise on treatment plans during the transition period to ensure continuity of care.
  • Utilize electronic medical record (EMR) of the community partners to document patient information, referrals or other relevant information as required.
  • Adhere to all compliance procedures relevant to protected health information (PHI) and HIPAA regulations.
  • Communicate clearly to community partners, care coordinators, or other designated contacts for the practices regarding limitations of consultation and treatment recommendations, if relevant.
  • Maintain communication flow in relevant systems with community partners and related Riverbend providers.
  • Provide on-site time at the MCHOC 12 hours per week.  On-site work may include meeting staff, care coordinators, or other designated staff, discussing procedures and coordinating services, providing education, and in-person evaluation of patients.
  • Provide on-site time at Community Bridges 4 hours per month.  On-site work may include meeting staff, care coordinators, or other designated staff, discussing procedures and coordinating services, providing education, and in-person evaluation of patients.

Requirements:

Education and/or Experience Requirements:

  • Master’s degree in Nursing (MSN)
  • Active NH APRN license
  • Board certification (or eligible) in Psychiatric/Mental Health Nursing or Clinical Nurse Specialist preferred
  • Valid DEA certificate with prescriptive authority
  • Valid driver’s license and personal vehicle or access to a reasonable alternative.
  • Excellent analytical, organizational, interpersonal, oral and written communication skills with attention to detail is required.
  • Experience with consultation and/or liaison psychiatric nursing is preferred.
  • Experience treating patients with a forensic history, diverse clinical profile and clinical co-morbidities is preferred.
