Case Manager, PDMS

Join our dynamic and energetic team of professionals committed to providing high quality Case Management services to adults (21+) in the self-directed services program, with developmental and/or intellectual disabilities and/or Traumatic Brain Injuries (TBI).

 

As a Case Manager, you will provide service coordination and assure the continuity of services for each client on your caseload. Additional responsibilities include: organizing, facilitating, and documenting service plans, and negotiating, monitoring, and evaluating the provisions of the individual’s services. This includes providing oversight and supporting families in managing their individual program budgets, as well as assisting families with staff supervision, as needed. You will assist clients with accessing resources and maintaining the highest level of community integration. Clear and open communication with the client and family is essential. Some of the most important skills successful Case Managers possess are: time management, autonomous decision making, ability to triage multiple high-need situations, interpersonal skills, and the ability to adapt to change. Case Managers in the self-directed services program work closely with the Business Office on financial matters specific to individual program budgets.

 

This 40-hours/week position offers a generous benefit package to staff including paid time off, health and dental insurance, retirement planning and opportunities for growth within the Agency.

Requirements:

An Associate’s Degree in a related field PLUS two years’ relevant field experience; or a Bachelor’s Degree in related field is required. Excellent math skills and budgetary experience are preferred. Exceptional verbal & written communication skills required.

As local travel is a requirement of this position, candidate must have a valid driver’s license, clean driving record, proof of adequate vehicle liability coverage, and reliable transportation.

All candidates must pass a background check which includes criminal and driving records.
