Enhanced Care Coordinator needed for the Family Support Program serving the Concord, NH area.  As the Coordinator you will provide care coordination for children from birth to age five and their families. Focus is on prenatal care and early childhood, parenting education and child development, including family support in the areas of substance abuse and addiction treatment programs. Coordinators provide home and community-based support to build and coordinate individualized services for the child and families at risk.  Additional responsibilities include: conducting specialized wrap assessments, identifying goals, social risk factors, family needs, visit coordination and preparation. Responsibilities include developing and implementing treatment plans as well as linking the family to resources in the community.

The ideal candidate will have the ability to work independently collaborating with client and staff to coordinate services.  Flexible schedule including some evenings in order to accommodate family needs. Other skills needed are the ability to complete paperwork timely and effectively as well as having strong verbal and face to face communication skills. Requirements:

 BA degree in Social Work, Education, Rehabilitation or related field with three years related experience working with infants, prenatal care or parent education is required. Master degree preferred.  The chosen candidates will also be required to submit to a criminal background and motor vehicle record check.  Must have a valid driver’s license and auto insurance which meets agency standards. Knowledge of Concord area medical and behavioral services is helpful.
