Nurse Practitioner- Hospice

Job Duties

Works with the Hospice Medical Director to gather, document and share clinical findings to determine continued hospice eligibility. Performs face to face visits for hospice patients. 


Registered Nurse with an advanced degree in nursing and/or health care administration with:

  • Current NP/APRN license in New Hampshire
  • 3-4 years of recent APRN experience in a hospice or palliative care setting
  • Experience in the area of pain and symptom management
  • Certification in Hospice and Palliative Care

Knowledgeable in the hospice philosophy and principles, with the ability to apply these concepts and philosophies to patient and family care. Must have excellent communication skills. The Nurse Practitioner must be able to practice an interdisciplinary approach to patient and family care. 

Work Schedule: Full time, 40 hours per week.
