Gateways Community Services is seeking a Service Coordinator (Case Manager) to join our Adult Custom Services program. In this role, the Service Coordinator will provide guidance and direction for participating families by assisting in the development of annual service agreements and allocated budgets, and supporting and referring families to resources.

The Adult Custom Services program offers adults (21+ years) with developmental and intellectual disabilities and their allies the opportunity to design and manage the supports and services that are meaningful to them. Then, working within the guidelines, the individuals and their allies determine and provide authorization for the use of their Medicaid dollars and allocated budgets for their individualized supports and services.

Job Description: The Service Coordinator works with each participating individual and their families to empower them to choose, manage, and lead the supports and services that are meaningful to them. In doing so, the essential job responsibilities are:

  • Maintain a caseload of participating families; provide exemplary customer service to all participants
  • Explain roles and responsibilities to participants and their allies
  • Assist participants to develop the annual service agreement and allocated budget
  • Support participants and families in finding resources and negotiating rates of service
  • Coordinate resources to provide participants training as needed in areas such as: developing, monitoring and spending their allocated budgets
  • Monitor the service agreement plans for success and assist in revisions as needed
  • Contact participants at minimum of monthly and conduct annual face-to-face home visits to develop, implement, and monitor plans and budget
Requirements:

Job & Education Requirements:

  • Bachelor’s Degree in related field such as Social Work, Human Services, Psychology, Family Studies, Business Administration, required
  • Experience working with families and/or children or adults in an educational, health care, or social service capacity strongly desired
  • Strong interpersonal and independent professional skills such as: organizational, time management, problem solving
  • Facilitative skills for group and project management
  • Strong written and verbal communication is a must
  • Experience with Excel, budgets, spreadsheets strongly desired; basic operational financial literacy
  • Proficient in Microsoft Windows and Microsoft Office suite
  • Must have reliable transportation, valid driver’s license, and applicable vehicle insurance for business travel
