Community Health Educator

Partnership for Public Health

The Partnership for Public Health mission is to improve the health and wellbeing of the greater Lakes Region through interorganizational collaboration and community and public health improvement activities.  The goal of public health is to improve the health of individuals, families, and the community through disease prevention, health promotion, and protection from enviornmental threats.

The Partnership for Public Health (PPH) is seeking an energetic and innovative individual to join our Partners in Community Wellness (PicWell) team.

The Community Health Educator will report to the Director of Substance Use Disorder Systems Integration and provide critical assistance to the PicWell Team as we work toward building stakeholder engagement and coordinating the implementation of our region’s young adult strategy. Key Strategy: Universal SBIRT (Screening, Brief Intervention, Referral to Treatment) Screening for young adults age 18- 25.

The Winnipesaukee Public Health Region is comprised of 15 communities including all of Belknap County, plus the Merrimack County communities of Franklin, Hill, Danbury and Northfield.

  • Associate's Degree in a relevant field, Bachelor's preferred
  • 1-2 years of relevant experience working in the field of public health and/or community-based socail work preferred
  • Knowledge of and experience working with different sectors (business, education, law enforcement, health care, community supports, government)
  • Familiarity with our region's 15 communities a plus
  • Must be a self starter
  • Ability to establish and maintain positive professional relationships
  • Must have ability to multitask, prioritize, and operate successfully in a high volume, fast-paced environment
  • Must have excellent communication skills both verbal and written; ability to provide close attention to detail, strong customer service skills and excellent team work
  • Strong computer skills using Microsoft Office
  • Coordinate events and meetings (occasional evenings and weekends)
  • Develop SBIRT outreach materials
  • Coordinate/ facilitate trainings
  • Provide Community Presentations on relevant data and emerging trends
  • Develop and maintain communication  with our existing regional partners, and cultivate new partnerships that support the implementation of SBIRT
  • Work with Office Manager to maintain Partners in Community Wellness website with current material and resources
  • Document outcomes and track successes
  • Attend local health fairs and events representing Partners in Community Wellness
  • Participate in ongoing trainings on substance misuse, SBIRT and other effective, evidenc- based prevention strategies targeted at young adults
  • Attend Regional Leadership Team meetings and participate on work groups as needed
  • Other tasks and duties
