Case Manager – Veterans FIRST Program

Full-time opportunity available for an individual committed to ensuring that all veterans and their families are residing in a safe facility.

As a Case Manager, you will provide direct services to veterans and other program participants by assisting with applications for housing resources and financial benefits such as SSI, SSDI, VA Benefits, Food Stamps and other applicable benefits.  This position involves maintaining a case load of approximately 20 clients who you will meet with a minimum of once a week to ensure that appropriate goals are set and clients are actively committed to the program.  You will conduct assessments and develop residential based service plans for all program participants as well as maintain documentation of interventions and services provided to them and any other documentation required by the Veterans Administration. In addition, you will conduct drug and alcohol tests with residents.  Assisting clients with transportation may also be necessary. 

Bachelor’s degree in human service field preferred along with experience, education and/or training in social work, occupational therapy, counseling, case management, nursing, psychology or other related human service field.  Valid Driver’s license required.   Excellent communication, organization and computer skills a must!
